DISCLAIMER: Premium payment is your responsibility, so please watch for your invoices. We will not follow up with you for non-payment of premium. If the carrier does not receive your payment by the due date, a late fee may be charged, and a Notice of Cancellation will be issued. If the policy should go into cancellation mode, for non-payment of premium, you will be without protection. To avoid any issues, you may wish to choose automatic payments option for making your payments.

NOTE: You cannot bind, alter or cancel coverage without speaking to an authorized representative of the Raymond A. Gralak Insurance Agency Inc. You cannot assume coverage is bound without written confirmation from an authorized representative of the Raymond A. Gralak Insurance Agency Inc or policy documents from the insurance carrier.

IMPORTANT AGENCY SERVICE NOTICE: All requests for coverage inception, cancellation, or change must be in writing from you and confirmed in writing by the carrier in policy documents. Quotes do not provide coverage until you complete an application, submit a bind request, pay the premium, and receive a policy. It is your responsibility to review all applications, change requests, and policy documents obtained from the insurance carrier or our office and notify our office or the insurance carrier of any errors. Failure to do so may cause uncovered loss for which my office will not be responsible. In addition, you are responsible for reviewing all correspondence and notices, whether sent directly from the insurer or our office. If not responded to within 30 days, any offer of coverage made by us is deemed declined, and you have found your current insurance coverage meets all your needs. Any insurance coverage you currently do not have with this office will require you to contact the office for availability. This notice is an offer of coverage that requires your action to implement, and your failure to do so constitutes acceptance and approval of your current coverage.
